If you're a Nammed member in need of chronic medication, we offer multiple convenient ways to submit your application:

Through Your Doctor:
- Your doctor can write a prescription or complete a chronic medication form to register your medication.

Via Your Pharmacy:
- Your pharmacy can send your prescription directly to be loaded onto our administrative platform.

In Person:
- Drop off your prescription with our Managed Care Department for processing.

By Fax or Email:
- Fax your prescription to +264 (0)61 374 620 or email it to info@nammed.org.na.

Online Submission:
- Fill in the form below, attach your chronic medication prescription, and submit your details directly to us.
Important Note:
If your chronic medication is not registered, it may deplete your day-to-day acute medication benefits. Ensure your chronic medication is loaded to avoid this.
